Description

BWC-3102 - TRANSITIONAL WORK GRANT AGREEMENT. The Bureau of Workers’ Compensation form is a contract between an eligible Ohio employer and the Ohio Bureau of Workers’ Compensation (BWC). This agreement outlines the terms under which the employer may receive a Transitional Work Grant (TWG) from BWC to offset the costs of developing and implementing a transitional work plan. The purpose of this plan is to help injured employees return to work more quickly through modified or alternative job duties. The grant is intended to cover expenses incurred by hiring a BWC-approved transitional work developer to create a customized program based on the employer’s specific workforce and job functions. Eligibility is limited to state-fund private employers and public employer taxing districts, and employers may only receive one grant per policy number every five years. The amount of the grant is determined by the number of employees, with funding caps ranging from $3,700 to $8,200. Employers must use the grant exclusively for covered services, such as transitional work developer labor and job analyses, within specified reimbursement limits. BWC retains the right to monitor the development and use of the plan, verify invoices, and approve all reimbursements. If the employer fails to implement the program within one year, misuses funds, or violates program rules, they may be disqualified and required to return the full grant amount. The agreement also contains compliance clauses regarding ethics, conflicts of interest, non-discrimination, and legal authority, and it certifies that the employer will use all funds appropriately and in good faith.
