I-800 - SUPPLEMENT 1, CONSENT TO DISCLOSE INFORMATION. This form is used by individuals filing a petition to classify a Convention adoptee as an immediate relative (Form I-800) with U.S. Citizenship and Immigration Services (USCIS). This supplement allows petitioners to authorize USCIS to share information about their case with their designated primary adoption service provider. Because the information contained in the petition is protected under the Privacy Act, USCIS cannot disclose it without the petitioner’s written consent. By signing this form, the petitioner gives USCIS permission to send notices and discuss case details—including the current Form I-800 and any previous Form I-800A proceedings—with the specified adoption service provider. However, this authorization does not make the adoption service provider the petitioner’s legal representative before USCIS unless the provider is specifically authorized under immigration law. The form also includes an option to withdraw any prior consent to disclosure previously given through earlier supplements.
