This form is used in the Vermont Superior Court, Probate Division, to initiate legal proceedings related to a trust established by a will (testamentary trust) or an inter vivos trust. The petitioner may request the court to appoint or replace a trustee, approve a nonjudicial settlement, or modify, terminate, or reform a trust under relevant sections of the Vermont Trust Code (14A V.S.A. §§ 411–415, 812). The form also allows requests for actions such as compelling property delivery, redressing trustee breaches, or approving trustee reports.
