AOC-SP-300 - AFFIDAVIT AND PETITION FOR INVOLUNTARY COMMITMENT. This form is used in the District Court Division of the North Carolina General Court of Justice to initiate the legal process of committing an individual, known as the respondent, to a mental health or substance abuse facility without their consent.
