Description
PR008 - ORDER APPOINTING COUNSEL FOR CONSERVATEE. This form is used in the Superior Court of California, County of San Luis Obispo, to formally appoint an attorney to represent a proposed conservatee or current conservatee in a conservatorship proceeding. This appointment ensures that the conservatee has legal representation to protect their interests, particularly when they are unable or unwilling to retain counsel themselves. The form grants the appointed attorney the authority to access and copy the conservatee’s medical records without their consent, in accordance with California law, to better evaluate and advocate for their needs. The court may order the attorney to submit a written report by a specific date and provides the legal basis for the appointment, such as when dementia powers are being sought, or when the proceeding involves establishing, transferring, or terminating a conservatorship. This appointment remains in effect until the court formally discharges the attorney. Form Adopted for Optional Use. www.FormsWorkflow.com
