CCJP 0653 - FINDING REGARDING PATERNITY. This form is used by the Child Protection Division of the Circuit Court of Cook County, Illinois, to officially establish or deny legal paternity of a minor in child protection proceedings. This form is completed after a hearing in which all involved parties are either present or represented by counsel.
