AOC-CVR-3 - REVOCATION ORDER WHEN PERSON NOT PRESENT. This form is used by a judicial official to revoke a person’s driver’s license or driving privilege when the person is not present in court and has been charged with an offense involving implied consent laws, such as driving while impaired (DWI). This form is based on probable cause that a law enforcement officer had reasonable grounds to believe the person committed an applicable offense and either refused a chemical test or had a qualifying alcohol concentration. It orders that the individual's license be revoked for at least 30 days, or 45 days if they fail to surrender their license or demonstrate that they are not currently licensed within five working days of the order’s effective date. If the person has multiple pending offenses, the revocation may be indefinite until all related cases are resolved. The form provides detailed instructions about how and when to surrender a license, pay the mandatory $100 restoration fee, and the right to request a hearing within 10 days to contest the revocation. It also outlines supplemental findings, procedures for license pickup, and acknowledges whether the license was returned or mailed.
