M-33 - TRANSCRIPT ORDER FORM. This form is used in the Superior Court of California, County of El Dorado to request a transcript of court proceedings from a court reporter. This form allows individuals to specify details such as the case name, case number, judge, department, and hearing dates for which they need a transcript. It also provides options to request the entire transcript, a judge’s order, specific testimony, or other portions of the proceedings. The requester must select the court reporter who recorded the hearing or allow court staff to determine the correct reporter. Once submitted, the court reporter will provide a cost estimate, and the transcript will be prepared after payment is received.
