By submitting this form, the self-represented party formally requests to be exempt from the court’s e-mail service requirement outlined in Florida Rule of General Practice and Judicial Administration 2.516. The form allows the individual to receive all legal documents, notices, and communications by mail or in-person delivery instead of electronically. It also requires the party to provide a valid mailing address and to keep both the clerk’s office and opposing parties informed of any changes to that address.
