MCR-103 - REQUEST TO CALENDAR CASE. This form from the Superior Court of California, County of Mendocino is used to formally ask the court to place a criminal case on the calendar for a new hearing date. This request can be submitted by the defendant, their attorney, a deputy district attorney, a probation officer, or a court clerk, and applies whether the defendant is in custody or out of custody. The form allows the requesting party to select the reason for the hearing, which may include a violation or modification of probation, a request to terminate probation, a request to modify sentencing terms such as extending time to pay a fine or complete community service, or changes related to DUI programs or ignition interlock requirements. It may also be used to request withdrawal of a plea, dismissal under Penal Code section 1203.4, recall of a bench warrant, or to advance an upcoming hearing. If the request involves a modification or other action that requires explanation, a declaration must be attached on the appropriate court-provided form. The court will not file or consider requests without the required declarations.
