LOCAL FORM 4004-1B283 (FORM 283) - CHAPTER 12 or 13 DEBTOR’S CERTIFICATIONS REGARDING DOMESTIC SUPPORT OBLIGATIONS AND SECTION 522(q). This form is used in the United States Bankruptcy Court for the District of Minnesota by debtors completing a Chapter 12 or Chapter 13 bankruptcy case. The purpose of the form is to provide two required certifications before the debtor can receive a discharge of their debts. In Part I, the debtor must certify whether they were ever required to pay a domestic support obligation, such as child support or alimony, and if so, confirm that all required payments have been made from the time of filing the bankruptcy petition through the current date. If the debtor has been required to pay such obligations, they must also provide their current address and employer information. In Part III, the debtor must certify whether they have claimed a homestead or similar exemption under Section 522(b)(3) that exceeds a certain dollar threshold, which is periodically adjusted.
