ATJ 140.1 - MOTION FOR DEFAULT (DIVORCE). This is an Illinois Supreme Court–approved form used by a petitioner to ask the court to hold the respondent in default for failing to file an Appearance, Answer, or Response after proper service. The form requires the petitioner to specify how and when the respondent was served—either personal service or substitute service at the home followed by mailing—and confirm that more than 30 days have passed since service. The petitioner then requests the court to enter a default.
