Form 9 IAR - INDEPENDENT AGENT ANNUAL REPORT. This is a form used by licensed gaming establishments in Nevada to report all independent agents with whom they had agreements during the reporting year. Required pursuant to NGC Regulation 25.040(2), the report must be submitted no later than February 15th of each year. The form includes fields for listing the account name of each independent agent, the location number (formatted as #####-01), and the date of the agreement relevant to that year.
