This form is used in the United States Bankruptcy Court for the District of Alaska by business entities—such as corporations, partnerships, or limited liability companies (LLCs)—that are filing for bankruptcy electronically through an attorney. The form serves as a sworn statement by the debtor’s authorized representative affirming that the information provided to their attorney, as well as the contents of the electronically filed bankruptcy documents (including the petition, schedules, statements, and related forms), is true and correct. The representative also gives consent for their attorney to electronically file these documents with the court. This declaration must be submitted to the court within 14 days of the electronic filing of the bankruptcy petition. If it is not filed on time, the bankruptcy case may be dismissed after a brief hearing.
