This form is used in Minnesota appellate courts, including the Supreme Court and Court of Appeals, by the prevailing party to request reimbursement for allowable legal expenses incurred during the appeal. This form allows the prevailing party—either the appellant, relator, or respondent—to itemize and claim costs such as statutory fees, printing expenses for briefs and addendums, transcript costs, court filing fees, and other disbursements related to the appeal. The form must be signed under oath by the prevailing party’s attorney, confirming the accuracy of the claimed costs. It also includes a section for the Clerk of Appellate Courts to certify the taxation of costs.
