§1-109 VERIFICATION BY CERTIFICATION. This form is used by Illinois law firms to demonstrate compliance with Supreme Court Rule 722 regarding minimum professional liability insurance or proof of financial responsibility. An authorized shareholder, member, or partner of the firm must certify—under penalties prescribed by §1-109 of the Illinois Code of Civil Procedure—that the firm maintains the required insurance coverage or financial reserves. This certification accompanies applications or renewals for a Certificate of Registration to engage in the practice of law as a professional corporation, association, LLC, or LLP.
