FBE - CERTIFICATE OF AUTHORITY (FOREIGN BUSINESS ENTITY). This form is used by foreign (out-of-state or international) business entities to obtain authorization to transact business in Kentucky, pursuant to KRS 14A-030. Applicants must provide their exact legal name as registered in their home jurisdiction, principal office address, registered agent and office in Kentucky, type of entity (e.g., corporation, LLC, partnership, trust), and details about officers, directors, managers, or trustees. The registered agent must consent in writing, and a Kentucky street address (no P.O. boxes) is required. The form also allows designation of an alternate name if the original is unavailable in Kentucky. Filing requires a $90 fee payable to the Kentucky State Treasurer.
