FORM 21.2 - APPLICATION FOR CHANGE OF NAME OF MINOR. This form from the Probate Court of Stark County, Ohio is used when a parent, legal guardian, legal custodian, or guardian ad litem seeks to legally change the name of a minor child. In order to file this application, the minor must have lived in Stark County for at least 60 days immediately prior to the filing. The applicant must provide both the minor’s current legal name and the new name being requested, along with a clear reason for the name change. A certified copy of the child’s birth certificate must be attached, and the form requires information about both parents (or the alleged father), including their names and addresses. If either parent’s address is unknown, the applicant must affirm that they have made reasonable efforts to locate them. If both parents do not consent to the name change, the court requires notice of the hearing to be served on any non-consenting parent or alleged father, as outlined in Ohio Revised Code 2717.14.
