CCTR 0506 - PETITION TO RESCIND STATUTORY SUMMARY SUSPENSION. This form is used by a driver in Illinois to challenge the automatic suspension of their driver’s license following an arrest for driving under the influence (DUI). The petitioner uses this form to formally request that the court overturn, or rescind, the suspension by presenting specific legal grounds for doing so.
