FORM 18.3 - CONSENT TO ADOPTION. This form from the Probate Court of Stark County, Ohio, is used to obtain legal consent for the adoption of a minor. It is required under Ohio Revised Code (R.C.) Sections 3107.06, 3107.08, and 3107.081 and must be signed by a legally authorized individual, such as a birth parent, putative father, legal guardian, adoption agency representative, or, in some cases, the minor being adopted (if over 12 years old). By signing this form, the consenting party waives their right to notice of the adoption hearing and voluntarily agrees to the adoption without requiring disclosure of the adoptive parents' identities.
