FORM 17.7 – GUARDIAN’S REPORT. This form used by the Probate Court of Stark County, Ohio is a required document that a court-appointed guardian must periodically complete to provide a detailed update on the status and well-being of the ward—the individual under guardianship. This report includes information about the ward’s current address, type of living arrangement, and the duration of their stay at that location.
