FORM 17.7 – GUARDIAN’S REPORT. This form used by the Probate Court of Stark County, Ohio is a required document that a court-appointed guardian must periodically complete to provide a detailed update on the status and well-being of the ward—the individual under guardianship. This report includes information about the ward’s current address, type of living arrangement, and the duration of their stay at that location. It also asks the guardian to report how often and in what manner they have been in contact with the ward, the date they last saw the ward, and whether there have been any significant changes in the ward’s physical or mental condition. The form seeks information on whether the ward has been seen by a physician, whether the care being provided is adequate, and if the guardianship should continue. The guardian must also certify whether they are serving as guardian for ten or more wards and confirm that they meet any continuing education requirements.
