DC 6:11.1 - PETITION FOR NAME CHANGE OF A MINOR CHILD OR CHILDREN. This form is used in Nebraska District Court by a parent or guardian to request a legal change of a minor child’s name. The form allows the petitioner to provide information about themselves, the minor child(ren), their current addresses, and their relationship to the child(ren). It identifies the other parent and lists the child(ren)’s current names as well as the preferred new names. The petitioner explains the reasons for requesting the name change and affirms that both they and the minor child(ren) have been residents of the county for more than one year.
