INITIAL GUARDIANSHIP PLAN (GUARDIANSHIP REPORT) OF GUARDIAN OF THE PERSON (adult). This form is used in the Circuit Court for Hillsborough County, Florida, within the Guardianship, Probate, and Mental Health Division. This form is required when an individual has been appointed as a guardian for an adult who has been legally determined to be incapacitated and in need of assistance in making decisions regarding their personal and medical care. The form outlines the guardian’s plan for managing the ward’s care during a specified reporting period.
