DC 6:5.27 - AFFIDAVIT AND APPLICATION FOR ORDER TO SHOW CAUSE (VISITATION). This form is used in Nebraska District Court by a non-custodial parent who believes that the custodial parent is willfully violating a court-ordered visitation schedule. The person completing the form, typically without an attorney, requests that the court issue an order requiring the custodial parent to appear in court and explain why they should not be held in contempt for denying court-ordered visitation. The applicant must provide details such as the date the visitation order was issued, describe how and when the custodial parent failed to comply, and affirm that the visitation order is still valid. The form must be signed under penalty of perjury and notarized.
