DC 3:02 - CHANGE OF CONTACT INFORMATION. This form is used in Nebraska courts to notify the court of updated personal contact details for a party involved in a legal case. It allows an individual—either a plaintiff or defendant—to provide their new mailing address, email address, and/or cell phone number, or to inform the court that they no longer have access to email or no longer wish to receive text message notifications about their case. The individual completing the form must sign it and acknowledge that the updated contact information will become part of the public record.
