APPLICATION FOR SUPPLEMENTARY PROCESS. This form is used in Massachusetts to initiate a supplementary process proceeding under General Laws Chapter 224, Section 14. This procedure is employed by a judgment creditor (plaintiff) when the judgment debtor (defendant) has failed to voluntarily pay a civil money judgment. The purpose of this form is to request a court hearing where the debtor will be examined regarding their ability to pay the judgment. The court may issue an order to compel the debtor to make payment. This form must be filed in the appropriate court (e.g., District Court, Boston Municipal Court, Housing Court) where the debtor resides or has a business. The creditor must complete the application and submit it along with the required entry fee. Once filed, the court will issue a summons to the debtor, which must be served at least 7 days before the hearing.
