TRADEMARK REGISTRATION INFORMATION CHANGE FORM. This form is used to update details for an existing registered trademark with the Utah Division of Corporations and Commercial Code. This form, which carries a $13 non-refundable filing fee, allows registrants to update the principal’s address, modify class codes for goods or services, add new classes (with an additional $25 fee per class), or include federal filing and usage information. It cannot be used to change ownership; ownership changes require filing a separate Assignment of Utah Trademark form.
