CR24-0011 - HEALTH AND SAFETY CODE § 11369 ADVISEMENT – ADDENDUM TO PLEA FORM. This form is used in the Superior Court of California, County of Humboldt, in criminal cases where a defendant pleads guilty or no contest to specific drug-related offenses under the California Health and Safety Code (§§ 11351, 11351.5, 11352, 11378, 11378.5, 11379, 11379.5, or 11379.6). The form ensures that the defendant acknowledges the serious dangers of manufacturing, distributing, selling, or administering real or counterfeit drugs. It explicitly warns that if such conduct results in someone's death, the defendant could face homicide charges, including murder under Penal Code § 187. The defendant must initial and sign the form to confirm understanding, and their attorney must also sign to affirm they have explained it.
