MDJ-009 - NOTICE OF ENTRY OF APPEARANCE FOR VICTIM OR VICTIM’S REPRESENTATIVE (Md. Rules 1-326(d) and 20-109(b)). This form is used in the Supreme Court of Maryland, Appellate Court of Maryland, Circuit Court, or District Court to formally notify the court that a victim or the victim’s authorized representative is participating in a criminal case. By submitting this form, the individual requests official recognition in the case and seeks access to all relevant case records, including electronic and remote access, as allowed under Maryland Rules 1-326(d) and 20-109(b). However, access excludes any records that are sealed, shielded, or protected by court order under Title 16, Chapter 900 of the Maryland Rules. This form is completed by the victim or their legal representative, who must provide their name, contact information, and signature. If represented by an attorney, the attorney may include their "Victim Attorney Number," which is used for identification purposes but is not entered into the court’s case management system.
