PETITION FOR CHANGE OF NAME OF ADULT (NO DISSOLUTION OF MARRIAGE). This form is used by an adult who is requesting a legal name change in the state of Iowa and is not doing so as part of a marriage dissolution (divorce). It is filed in the Iowa District Court and governed by Iowa Code chapter 674. The petitioner provides their current full legal name, physical description, birth information, and current and past addresses. The form also asks for information about marital status, the names of any affected minor children, and the reason for the requested name change. If the petitioner is married, they must notify their spouse of the petition. The form requires the submission of a certified copy of the petitioner’s birth certificate or, if that is unavailable, another form of acceptable identification.
