CERTIFICATION COVER SHEET. This form is used in the Court of Common Pleas of Lancaster County, Pennsylvania is a required document that must be filed in all residential mortgage foreclosure actions. Its purpose is to certify whether the subject property qualifies for participation in the Residential Mortgage Foreclosure Diversion Program, which was established by administrative order A.D. No. 27-2021 and related orders.
