1150.5FAJ-C - EX PARTE VERIFIED MOTION TO ENFORCE DOMESTIC ORDER AND FOR SANCTIONS - COMMISSIONER. This form is used in the Utah District Court when a party seeks to enforce an existing domestic court order, such as those involving alimony, child support, custody, parent-time, property division, or debt obligations. This form allows the petitioner or respondent to identify the specific order that has not been followed, detail the noncompliance (such as unpaid support, unreimbursed expenses, failure to transfer property, or violations of custody or parent-time provisions), and request the court to enforce compliance. The motion can also request sanctions against the noncompliant party, reimbursement for costs incurred in enforcement, and in some cases, a finding of contempt.
