SCPC FORM 302 – ENTRY ORDERING INDEPENDENT HOME STUDY. This form is used by the Probate Court of Shelby County, Ohio, in matters involving the proposed placement of a child for adoption. When an individual files an application for placement, the court may order an independent home study to assess the suitability of the prospective adoptive home. This entry directs a designated agency to conduct the home study in accordance with Ohio Revised Code sections 5103.16(D)(2) and 3107.031. The agency must evaluate the proposed placement and submit a written report of its findings to the court. The cost of the home study is the responsibility of the person seeking to adopt the child.
