FORM 2 - ARTICLES OF INCORPORATION BUSINESS CORPORATION. This form is used to establish a domestic business corporation in the State of Wisconsin under Chapter 180 of the Wisconsin Statutes. This form is filed with the Wisconsin Department of Financial Institutions and requires basic corporate information, including the proposed name of the corporation, the number and types of shares authorized to be issued, the name and physical address of the registered agent and office located in Wisconsin, and the names and addresses of all incorporators.
