FORM 621 - FOREIGN REGISTRATION STATEMENT LIMITED LIABILITY PARTNERSHIP. This form is used to register a foreign limited liability partnership (LLP) with the State of Wisconsin Department of Financial Institutions. This mandatory filing enables a partnership formed under the laws of another U.S. state or foreign country to legally transact business within Wisconsin. The form collects essential details such as the foreign LLP’s name, jurisdiction of formation, registered agent and office in Wisconsin, and the principal office address.
