ILLINOIS RESIDENTIAL TRANSFER ON DEATH INSTRUMENT REVOCATION § 755 ILCS 27/1 ET SEQ. This form is used to formally cancel a previously recorded Transfer on Death Instrument (TODI). This allows a property owner to revoke the automatic transfer of real estate to designated beneficiaries upon their death. The form includes the owner's name, address, legal property description, and details of the original TODI, including its recording date and document number. The revocation must be signed by the owner(s) in the presence of witnesses and a notary public to be legally valid.
