PR-REP3 - ANNUAL REPORT. This form is used in the Circuit Court of the Twenty-Second Judicial Circuit in McHenry County, Illinois, in the Probate Division for the annual reporting of the guardianship of a disabled person. The guardian, appointed by the court, submits this Annual Report providing an update on the disabled person's condition, living arrangements, medical, educational, and vocational services, as well as the guardian's visits and activities on behalf of the ward.
