147 - SUMMONS 5 DAY CORPORATE SERVICE (SUMMARY PROCEDURE PURSUANT TO F.S. 51.011). This form is used in either the Circuit Court of the Eleventh Judicial Circuit or the County Court in Miami-Dade County, Florida. It is intended for cases where the Summary Procedure is being followed pursuant to Florida Statute 51.011. The purpose of this summons is to officially notify the defendant(s) in a legal action about the lawsuit filed against them and initiate a summary procedure that allows for a quicker resolution of the case. The form includes options for the division of the court (civil, districts, other) and provides the case number. It identifies the plaintiff(s) and the defendant(s) or respondent(s) involved in the case. The summons is addressed to each sheriff of the state, instructing them to serve the summons and a copy of the complaint or petition on the named defendant(s) or respondent(s). The defendant(s) or respondent(s) are required to serve a written defense to the complaint or petition within a significantly shorter time frame of 5 days from the date of service. This time period excludes the day of service, Saturdays, Sundays, and legal holidays.
