PRO-009 - TERMINATION OF GUARDIANSHIP QUESTIONNAIRE (CONFIDENTIAL). This form is used in the Superior Court of California, County of Tulare. It is intended to be completed by the parent(s) or other party who will have custody of the child if the guardianship is terminated. The purpose of the questionnaire is to gather information about the child, the petitioner filing the petition to terminate guardianship, and the living arrangements and circumstances surrounding the potential termination of guardianship.
