This form is used in Georgia civil cases to formally notify the court and opposing parties that a party is appealing a judgment or order to the Court of Appeals of Georgia. This form must be filed in the trial court that entered the order or judgment, not in the Court of Appeals. The notice identifies the appealing party, the order or judgment being appealed, relevant post-judgment motions, whether a transcript will be filed, and the basis for appellate jurisdiction. The form also allows the appellant to designate items to be omitted from the record on appeal.
