This form is used to authorize the Florida Department of Children and Families (DCF) to conduct a background check for any confirmed reports of abuse, neglect, or exploitation related to a guardian applicant. It is typically required in guardianship proceedings—particularly for family or non-professional guardians—when a person seeks appointment as guardian over another individual (the “ward”). The applicant must provide personal identifying information, including full legal name, prior names, race, sex, date of birth, Social Security number, and all Florida residential addresses from 1978 to the present. This background check ensures that potential guardians do not have a history of abuse or misconduct that would disqualify them from serving. Once completed, the form must be submitted to the Administrative Office of the Courts for review and processing.
