Description
CRM-051 - APPLICATION FOR REDUCTION TO MISDEMEANOR – RESPONSE AND ORDER (Penal Code §1170.18(f)). This is a Superior Court of California form used in Alameda County to request that eligible felony convictions be redesignated as misdemeanors under Penal Code § 1170.18(f) (commonly associated with Proposition 47 relief). This application is filed by an individual who has completed their sentence and asserts eligibility for misdemeanor designation of specified felony count(s). The form includes sections for the applicant’s sworn declaration, consent to electronic service, and whether a hearing is requested. It also provides space for the District Attorney’s response and a court order granting or denying relief, or setting the matter for hearing. The applicant must acknowledge that firearm prohibitions may still apply even if relief is granted. Adopted for Mandatory Use. www.FormsWorkflow.com
