Description
SFUFC-11.9-B - DECLARATION REGARDING ACCEPTANCE OF SERVICE AT "IN CARE OF" ADDRESS. This is a San Francisco Unified Family Court form used when a petitioner or respondent wants to use a mailing address listed “in care of” (c/o) another person or location instead of a home or office address in a family law case. The form explains why the party cannot provide a residence or business address and confirms that the filer understands California service-by-mail requirements under Code of Civil Procedure section 1013. By signing the declaration, the party agrees to accept legal service related to the case at the designated c/o address. The form must be signed under penalty of perjury. www.FormsWorkflow.com
