ARIZONA SUPERIOR COURT, PIMA COUNTY SELF-SERVICE PACKET INSTRUCTIONS FOR REQUESTING CONCILIATION General: The Family Center of the Conciliation Court was established to preserve, improve and protect family life through individual and family counseling, child custody/parenting time evaluations, mediation and community education. Counseling is confidential, usually short-term, and crisis oriented. Counselors have professional degrees in the behavioral sciences with experience in mediation and marriage and divorce therapy for individuals, couples, and families. Their services are available at no charge to any resident of Pima County and services are available for Spanish-speaking families. Either party in a family law case may file a Petition for Conciliation counseling. This requires both husband and wife to attend at least one session at the Family Center of the Conciliation Court to help couples gain a better understanding of the problems in their marriage and consider more workable solutions. Once the divorce is granted, a petition for mandatory Conciliation Court counseling cannot be filed. While a Petition for Conciliation is pending, the divorce or legal separation cannot be made final, but the Court can make temporary orders for custody, parenting time and support. This rule only applies if another Petition for Conciliation was not filed in the past 12 months. Complete the Petition for Conciliation in the following manner: PLEASE PRINT CLEARLY your Superior Court Case Number, if a divorce, legal separation or annulment. Complete the caption of the case. If you are the person requesting the services of the Conciliation Court, you are the Petitioner, regardless of who was the petitioner in the underlying divorce, legal separation or annulment action. The non-requesting party is the Respondent in this Petition. Also, list as additional Respondent(s) the name of any other party to this controversy. In paragraph 2, list the name and age of each minor child whose welfare might be affected by the controversy. Use additional pages if there are more than four (4) children. If there is already a divorce, legal separation or annulment case filed in this matter, check the appropriate block in paragraph 3 and give the county and case number of the case and check whether you are the Petitioner or the Respondent in that case. Check the appropriate block in paragraph 4 if a Petition for Conciliation has been filed previously in this matter. In paragraph 5, list your address and home and work telephone numbers. If you are a victim of domestic violence and you wish to keep your address and phone numbers confidential, you may use a P.O. Box number or a friend's address. In paragraph 6, list your spouse's address and home and work telephone numbers. In paragraph 7, list the name, address and home and work telephone numbers of any other person who has a relationship to this controversy and who you have named as a Respondent in the caption above. Date and sign the Petition and list your attorney's or your spouse's attorney's name and address and telephone number, if applicable. Make at least two (2) copies of this form. You must file this Petition with the Clerk of the Court and a copy of the Petition must be served on the Respondents. There is no filing fee for this Petition. If there is already a family law case filed and your spouse was served with the Petition in that case, you can mail a copy of this Petition to your spouse. Otherwise, this Petition will have to be formally served on your spouse.
