SDSC PR-064 - GUARDIANSHIP QUESTIONNAIRE – ESTATE (CONFIDENTIAL). This is a confidential form used in the Superior Court of California, County of San Diego, in guardianship of the estate cases involving a minor. This questionnaire must be completed separately by each proposed guardian. It collects detailed information about the minor, parents, custody arrangements, and the proposed guardian’s background, employment, financial history, assets, liabilities, and prior fiduciary experience. The form also requires disclosure of bankruptcy history, financial delinquencies, criminal convictions involving financial matters, and the expected value of the minor’s estate.
