This form is used by attorneys seeking general admission to the bar of the United States District Court for the Central District of Illinois under Local Rule CDIL-LR 83.5. Effective May 17, 2021, applicants must submit this completed form and any supporting documentation through their upgraded PACER account under “Attorney Admissions and E-File.” The form provides three paths for admission: (A) upon motion by an existing member of the court’s bar, (B) self-motion with a current Certificate of Good Standing, or (C) reciprocal admission for attorneys already admitted to another District of Illinois court.
