PRB-PGN-002 - CONFIDENTIAL REQUEST TO END GUARDIANSHIP. This is a confidential San Francisco Superior Court probate form used to ask the court to terminate an existing guardianship of a minor. The person requesting termination must provide detailed information explaining why ending the guardianship would be in the child’s best interest, who currently cares for the child, and who will assume care if the guardianship ends. The form also requires disclosure of living arrangements, household members, criminal history, child protective services involvement, weapons in the home, parental agreement, and whether the child wants the guardianship to end.
