Form 86 - AFFIDAVIT FOR WAGE DEDUCTION ORDER. This form is used in the Circuit Court for the Seventh Judicial Circuit of Illinois, Sangamon County, for matters involving the collection of a judgment through wage deductions. The form is completed by the judgment creditor or their attorney to state that an employer owes wages to a judgment debtor that can be used to satisfy an outstanding judgment. The form requires the creditor to provide the debtor’s last known address, the employer’s information, and a certification that the debtor was notified by mail of the wage deduction proceedings. It also includes a certification of the judgment amount, allowable costs, and the remaining balance owed.
