GAC 27-U - ORDER TERMINATING GUARDIANSHIP AND DISCHARGING GUARDIAN; CONSERVATORSHIP AND DISCHARGING CONSERVATOR. This form is used in the state of Minnesota to terminate a guardianship or conservatorship and discharge the guardian or conservator from their duties. It follows a hearing where the court considers whether the individual under guardianship or conservatorship has regained the capacity to make decisions about their personal care, or manage their property and affairs. The order officially terminates the guardianship or conservatorship, restores the individual’s rights, and discharges the appointed guardian or conservator.
