ORDER CHANGING NAME FOR A FAMILY. This form is issued by the Superior Court of Arizona in Mohave County that approves a request to change the legal names of multiple individuals within a family unit. This order is issued after the court determines that it has jurisdiction, finds good cause for the name change, and concludes that the change is in the best interest of any minor children involved. The form lists each person whose name is being changed, including their current legal name or name on their birth certificate, date and place of birth, and the new legal name to be adopted. If any of the individuals were born in Arizona, the court orders the Arizona Office of Vital Records to amend their birth certificates accordingly. For individuals born in other states, the order requests that the respective out-of-state agency honor the change if permitted by their laws.
